The mechanics differ by lock. Dial combinations change with a change key through the back of the lock body, following the exact procedure the lock was engineered around. Get it wrong and the safe locks with no valid combination at all, which is why dial changes belong in trained hands. Electronic codes reset through the keypad, and where the model supports it, old user codes and factory defaults get cleared out while we're in there.
The cardinal rule of combination work: everything happens, and gets tested, with the door open. New combination set, run several times, old code confirmed dead, all before the door ever swings shut. You never get locked out by your own combination change; that is the entire discipline, and it's why "tested with the door open" is on the checklist instead of in fine print.
When to do it is simpler: any time you can't name every person who knows the code. A newly bought safe (someone set that code before you), staff turnover (deposit-safe codes outlive employees), or a code that has quietly spread. A combination change is the safe-world equivalent of a rekey: same hardware, fresh start on who gets in.
